| In order to make the most of your WIA eligibility assessment please refer to the following readiness checklist prior to your scheduled appointment. You can make and appointment to meet with a caseworker on Wednesdays at the Transition Center. Please contact the Transition Center at 419-755-5371 to schedule your appointment. |

RESUME Please bring a printed copy or bring your resume on a flash drive or CD and we can print it for you at the Transition Center.
You can come in any time on Monday, Tuesday, or Thursday to create or update your resume between 8:00 a.m. - 4:00 p.m.
We ask that if you have a resume that you bring it to the center on a flash drive or CD so we can assist you in updating it. If you are just getting started please gather all relevant information about your career and a guest staffer can create a resume with you.

FAFSA (applicants are required to apply for FAFSA)

TABE (All Applicants must complete a Test of Adult Basic Education)
25 Minute Locator (This will determine the level of TABE test you will receive) 25 Minute Reading Section 15 Minute Math Computation 25 Minute Applied Math
TABE testing is done on Mondays & Wednesdays from 8:00 a.m. to 4:00 p.m.

ACTUAL INTERVIEW / ASSESSMENT (Typically takes 1 Hour) Please contact the Transition Center at 419-755-5371 to set up an appointment.
Interviews / Assessments are conducted on Wednesdays only from 8:00 a.m.- 4:00 p.m.
During your Interview / Assessment the caseworker will gather information about you and your desired career path. Forms will be printed that you will need to sign.

After your assessment make sure you check with your caseworker to see if there is an additional information that you need to provide them in order to expedite the processing of your request.
